Of all the things people fear about growing older, losing your memory – your sense of who you are – sits near the top of the list. And it's a fear science has struggled to answer. Decades of drug development have produced treatments that, at best, offer modest and temporary relief.

So it's worth paying attention when researchers turn their focus to something far simpler: the oil already sitting on your kitchen counter.

A new systematic review and meta-analysis, published in early 2026 in Clinical Nutrition ESPEN, is the first of its kind to pool the clinical-trial evidence on extra virgin olive oil, dementia, and cognitive decline in older adults. The verdict is cautious but genuinely encouraging – and for anyone who cares about which olive oil they use, the details are where it gets interesting.

What did the researchers actually study?

A team led by Victor Cordeiro Simão at the Federal University of Goiás, working with collaborators at Yale and University College London, set out to answer a question that, surprisingly, no one had rigorously quantified before: does EVOO measurably help older adults facing mild cognitive impairment (MCI) or dementia?

To find out, they searched nine major scientific databases, screened 937 records, and applied strict criteria – only randomized controlled trials, the gold standard of clinical evidence. Five trials made the cut, conducted between 2014 and 2022 across Italy, Greece, Spain, and the United States. Together they included 747 older adults (average ages ranging from about 65 to 75).

The researchers then pooled the results using standard meta-analytic methods, graded each trial for risk of bias, and rated the overall quality of the evidence.

Does olive oil actually help with memory and cognition?

Across all five trials, EVOO was associated with a statistically significant improvement in global cognitive function. Breaking it down by the specific tests used:

  • Mini-Mental State Examination (MMSE): a mean improvement of 0.42 points favoring EVOO (p = 0.002)
  • Clock Drawing Test: a mean improvement of 0.47 points (p = 0.004)
  • Alzheimer's Disease Assessment Scale (ADAS-Cog): a 1.45-point improvement favoring EVOO (p = 0.007)

Notably, all five trials found EVOO effective on at least one cognitive measure – regardless of the dose used or the oil's polyphenol level. And the statistical "noise" between studies (heterogeneity) was low, meaning the trials largely pointed in the same direction.

For the first quantitative synthesis ever done on this question, that's a meaningful signal.

Is a 5-study review too small to trust?

Whenever a nutrition study makes headlines, the reflex critique is "small sample size." It deserves a fair hearing here – and a fair answer.

Yes, five trials is a modest number, and the authors say so plainly. They rated the overall certainty of the evidence as "low" and were careful to call the findings preliminary. That kind of candor is exactly what you want from good scientists, and we'd rather quote it than bury it.

But "small" is relative. We're talking about 747 people across five separate randomized controlled trials run by independent research groups on three continents – not a handful of volunteers in a single lab. When independent teams, using different oils and different methods, keep arriving at the same directional result, that consistency carries weight that no single study can. This review is the first to pull that scattered evidence into one place and show it holds together statistically.

There's one more piece of honesty worth flagging: the size of the average benefit, while real, was small – smaller than the threshold researchers consider clinically dramatic on a test like the MMSE. In other words, EVOO is not a drug, and no one is claiming it reverses dementia. What the evidence supports is subtler and, arguably, more useful: a food you can eat every day for the rest of your life appears to nudge the aging brain in the right direction, with none of the downsides of a pill.


Why polyphenols are the part that matters

The compounds credited with EVOO's neuroprotective effects aren't the fat itself – they're the polyphenols: bioactive molecules like oleocanthal, a natural anti-inflammatory, and hydroxytyrosol, a potent antioxidant. In laboratory and animal studies, these compounds appear to fight the exact processes that drive cognitive decline: neuroinflammation, oxidative stress, and the buildup of amyloid-beta plaques. Some research even suggests EVOO helps protect the blood–brain barrier and supports healthy blood flow to the brain.

The catch: polyphenol content varies enormously from one bottle to the next. In this review, the oils ranged from 280 mg/kg of polyphenols at the low end all the way up to 965 mg/kg at the high end – a more than threefold difference. A supermarket oil that's been heavily refined, aged, or heat-treated can lose most of these compounds entirely. The four most instructive trials, up close

The five studies were genuinely different from one another – in dose, duration, and design – which is part of why their agreement is compelling. Here are the four most instructive.

1. MICOIL (Tsolaki et al., 2020, Greece): high-phenolic oil, head to head

This is the trial that speaks most directly to the polyphenol question. Rather than simply comparing olive oil to no olive oil, the Greek researchers pitted high-phenolic EVOO against moderate-phenolic EVOO – and a Mediterranean diet control – in older adults with mild cognitive impairment, at a dose of 50 mL/day over one year.

The high-phenolic group came out ahead on the widest range of measures: MMSE, ADAS-Cog, Clock Drawing Test, and more. When the only variable being changed is how much polyphenol is in the oil, and the higher-polyphenol group does better, that's about as clean a signal as nutrition research offers that the phenols are doing the work.


2. Kaddoumi et al., 2022 (USA): the highest-polyphenol oil, versus refined oil

The American trial is the other standout for phenol-focused readers, and the review singles it out (alongside MICOIL) as one of the only two trials that precisely standardized the oil participants received.

It used the highest-polyphenol oil of any trial in the review – 965 mg/kg – at 30 mL/day for six months, and crucially compared it against refined olive oil, which has had its polyphenols largely removed. Participants on the real EVOO showed improvement in clinical dementia rating, and separate analysis found EVOO improved the function of the blood–brain barrier – a mechanistic clue to how the oil might protect the brain. Comparing phenol-rich oil to phenol-poor oil is the ideal way to isolate what polyphenols actually contribute, and the phenol-rich oil won.


3. Mazza et al., 2018 (Italy): you may not need much

If MICOIL and Kaddoumi make the case for high-quality oil, the Italian trial makes an encouraging case about quantity. It used the lowest dose and lowest polyphenol level in the entire review – roughly 22 to 33 mL/day (about two to three tablespoons) at 280 mg/kg – added to a Mediterranean diet over one year.

Even so, participants improved on both the MMSE and the ADAS-Cog. The takeaway is practical and reassuring: you don't need to drown your meals in oil. A modest, consistent daily amount showed measurable benefit.


4. PREDIMED-Navarra (Martínez-Lapiscina et al., 2013, Spain): the long game

The largest and longest trial in the review followed older Spanish adults for 6.5 years, assigning them to a Mediterranean diet enriched with EVOO (about 143 mL/day across the group), a Mediterranean diet with nuts, or a low-fat diet. The EVOO group scored better on both the MMSE and the Clock Drawing Test.

Its real lesson is about time. Neurodegeneration unfolds over decades, and this trial's long horizon is a reminder that EVOO's benefits appear to depend on sustained, everyday use – not a short-term cleanse.

(A fifth trial, PREDIMED-Barcelona (Valls-Pedret et al., 2015), followed a similar Mediterranean-diet-plus-EVOO design over 4.1 years and found EVOO linked to better memory and executive-function scores.)


How to use olive oil for brain health

Reading the evidence honestly, here's what holds up:

  • Consistency beats intensity. Benefits showed up at doses as low as ~2 tablespoons a day. The trials that ran longest saw the most reliable effects. This is a daily habit, not a quick fix.

  • Quality is the variable you control. The oils in these trials ranged more than threefold in polyphenol content, and the phenol-focused trials produced some of the most striking results. A high-phenolic oil is how you make sure you're getting the compounds that matter.

  • Use both cold and hot.

    Switching to olive oil (specifically high-phenolic olive oil) for all of your everyday cooking is a simple way to get the health benefits of polyphenols.
    While high heat degrades polyphenols slightly, starting with a high-phenolic olive oil means that high amounts of polyphenols are still present after exposure to heat. Cooking with olive oil is safer than using seed oils and studies have shown that polyphenols from olive oil transfer to the food.  


The honest bottom line

This review doesn't prove that olive oil prevents dementia, and it doesn't claim to. The effect sizes were modest, the certainty of evidence was rated low, and larger, longer trials are needed. EVOO is a complement to good medical care and a healthy lifestyle – never a substitute.

But it's also the first time the clinical evidence has been gathered, weighed, and found to point the same way: toward a simple, food-based habit with real biological plausibility and no downside. In a field crowded with disappointing supplements, a delicious oil that has protected Mediterranean brains for millennia is a rare and welcome piece of good news.

The science is still developing. But if you're going to make olive oil part of your daily life anyway – and the evidence keeps suggesting you should – it's worth making it an oil rich in the very compounds the researchers are pointing to.
